To get a similar quiz, you need to assign keywords (A, B, C, D, etc.) to the corresponding answers. After that, you need to set down a keyword-based message for each option. The results message is based on the majority of the selected answers by the user and will show the possibly appropriate message to him/her on the results page of the quiz. You can navigate through answers by pressing Tab or Shift+Tab, tick an answer by pressing Space,and move forward to the next question using the Enter key.
Here is an example of quiz created with points. There will be 10 questions, worth 1 points each. You either receive 1 point for each correct answer or -0.25 point for each answer that is NOT correct.

Then, go back to your quiz, and paste the variable in the textarea of the Result message. (Results Settings tab)
Regarding the Radar chart, unfortunately, the plugin does not have that kind of functionality.
Regarding the average result, you can make use of the %%avg_score%% variable. Again, you need to paste it in the Result Message textarea.

How can we import quizzes in bulk using Excel or CSV files?
